Once completed the form should be sent to: The membership secretary, Sir Gervase Lucas' Company, 10 Hebb Street, Worcester, Worcestershire, WR1 3HT. including a SAE for the return of your membership card(s) and a cheque or Postal Order for the correct type of membership being applied for (cheques should be made payable to Sir Gervase Lucas).
